Uzma Jafri, MD, CMD,

grew up in Houston, Texas and has lived in the West Valley for 13 years. Dr. Jafri is double board certified in family medicine and geriatrics, trained at Banner Good Samaritan, now BUMC. She worked as a hospitalist for 6 years specifically in the Sun Cities for geriatrics. Since 2011, she provides black bag doctor service for seniors 65 and older at bedside no matter where on the longterm care spectrum they live. She is adjunct faculty at BUMC Geriatric Fellowship program and trains physicians in geriatric medicine, focused on providing evidence based, responsible and sensitive longterm care. Dr. Jafri is a certified medical director, overseeing infection, safety and quality control at an assisted living facility for over 6 years, She has also been a medical director at Hospice of the West for 5 years and continues to serve in this capacity, often providing the Islamic point of view to chaplains serving Muslim patients. As such, her expertise in geriatric life runs from independent living to end of life care. In her spare time, she provides free full spectrum adult family medical care to uninsured patients at Phoenix Allies for Community Health, serves grassroots local organizations and refugee resettlement agencies’ pregnant populations, and is a certified speaker at Islamic Speakers Bureau of Arizona, providing multiple presentations on Islam across the Valley. Dr. Jafri is a practicing Muslimah, mother to 4 homeschooled children, wife of 14 years, a podcaster at Mommying While Muslim, and an advocate for seniors who routinely suffer ageism and culturally insensitive care. She is fluent in English, Spanish, and Urdu.

Saba Farooqi,

with care and compassion, has been in the healthcare industry for a combination of 20 years. She holds an Assisted Living Managers License from the AZNCIABOARD, and is a Certified Senior Advisor, CSA with the Society of Certified Senior Advisors. In 2003 Saba and her husband built their own Assisted Living health care facility to service the senior population, with a particular niche for Muslim seniors and other faiths so that their religious and cultural needs would not be ignored. As co-owner/Executive Director of 3 licensed assisted living care homes in Gilbert, Saba was blessed to have managed and helped many residents from supervisory, directed, to end of life hospice care. Saba’s education has been in Business and Public Administration in the UK with careers in hospitals and local government administration. In the US she pursued psychology and worked in the management sector of non-profit organizations. Through the years, Saba has dedicated some time to volunteer for causes and served on many boards, and as a President of American Muslim Women’s Association AMWA. She co-founded the MAA program to engage senior Muslim women in monthly meetings and activities, to elevate their social and educational wellbeing. Currently Saba is working on a new business/service venture as a partner of SR Senior Solutions LLC, specializing in placements and consulting services. She is a Program Chair for the Society of Certified Senior Advisors (AZ Leaders Network). Saba is excited to newly volunteer as Vice President of Arizona Muslim Senior Services, where she can further help seniors who are lacking in services that meet their specific cultural needs, while working towards pioneering programs for those who are to follow. In her spare time Saba pursues a variety of self-interests and enjoys life in sunny AZ with her family and friends.

Usman Khan

was born and raised in Detroit, Michigan and attended Wayne State University. He has more than 10 years of business marketing experience with a passion for geriatric care. He has been working in various roles within the healthcare industry since 2008, consistently meeting clients’ and their families’ needs and expectations. In 2012 Mr. Khan co-founded A-Plus Home Health Services. A Plus HHS is a skilled medical home health agency that has been serving the growing geriatric population in the East valley since 2013. He enjoys spending time with his two young kids, traveling, and playing tennis. He also has a strong passion for cooking and loves experimenting with new recipes.

Humaira Ameer

is a resident of Chandler, Arizona and has called the Phoenix metro area home since 1995. She is a board certified clinical pharmacist specializing in geriatrics and has managed a team of 20 pharmacists for a national Medicare and Medicaid Managed Care provider. Dr. Ameer previously served as Treasurer for American Muslim Women’s Association (ANWA) for 4 years. She co-founded ICEV Sunday Islamic school and later served as its principal for 2 years. As Treasurer of Arizona Muslim Senior Services, she is responsible for maintaining financial records, cash flow analysis, reconciliation of bank statements, bill payment and preparation of monthly income and expense statements for the board to review. She enjoys reading, gardening, cooking, traveling, and spending time with her family and friends.

Altaf Rajabally

grew up in Karachi, Pakistan and has lived in Phoenix metro since 2001. After receiving his degrees in both accounting and finance in Pakistan and America, respectively, he worked as an Auditor and Finance Controller for major retail companies. 

He became Director of Finance at American Express Pakistan and served as such for 10 years prior to transferring to the US to manage its internal finances of its largest branch based out of Phoenix, Arizona. In 2007, Mr. Rajabally and his wife, Fauzia founded Arizona Care Providers to bring home care to people with developmental disabilities. Licensed by the State of Arizona, their startup has grown to serve Phoenix metro with offices in Phoenix, Peoria and Mesa. They employ numerous trained caregivers providing direct client care, as well as all a knowledgeable administrative staff managing these specialized services. He enjoys cycling and woodworking,
